Perian 1.1: Understanding the Epidemiology and Management of Penile Fracture in Iran

Penile fracture, also known as penile rupture or penis shaft fracture, is a rare but potentially serious injury that requires immediate attention. According to recent studies, prompt surgical intervention can lead to excellent functional results with minimal morbidity and short hospital stays. In this article, we will delve into the epidemiology and management of penile fracture in Iran, highlighting the challenges and recommendations for healthcare providers.

Diagnosis and Incidence

The majority of reviewed studies relied on patient history and clinical examination to diagnose penile fracture, with imaging only recommended in cases where urethral injuries are suspected. Interestingly, the incidence of penile fracture varies significantly across different geographical areas, with urologists practicing in Iran reporting an average of one case every 3.5 months.

Our study estimated the incidence of penile fracture in Iran to be between 1.14 and 10.48 per 100,000 male population, with a likely lower end estimate closer to the lower bound. Notably, the western province of Kermanshah had a significantly higher rate of penile fracture compared to other regions.

Challenges and Recommendations

Despite its relative rarity, penile fracture poses significant challenges in diagnosis, management, and reporting. Our review of Iranian medical literature found that there are no accurate data on the incidence of penile fracture nationwide. Furthermore, case series have reported considerable incidence rates in certain parts of Iran, underscoring the need for more comprehensive epidemiological studies.

To better understand the scope of this issue, we conducted a questionnaire-based study, sending surveys to 700 urologists across the country and receiving a 14% response rate. This study highlights the importance of accurate data collection and reporting in penile fracture management.

In conclusion, penile fracture is an emergency in urology that requires prompt attention. While there are challenges in diagnosing and managing this condition, our review of Iranian medical literature emphasizes the need for more comprehensive epidemiological studies to better understand its incidence and prevalence across different regions. By working together, healthcare providers can improve patient outcomes and ensure timely and effective management of penile fracture cases.
